Perio Plan – $425/year
Includes the following services for 12 months:
- Comprehensive or periodic oral exam (2 per year)
- Full-mouth or bitewing X-rays (when due)
- Periodontal maintenance cleaning (3 per year)
- Topical fluoride varnish (3 per year)
- Limited exam with 1 X-ray (1 per year)
- 10% courtesy on basic and major dental procedures
(For patients who have completed deep cleaning and require periodontal maintenance)
Prophy Plan – $250/year
Includes the following services for 12 months:
- Comprehensive or periodic oral exam (2 per year)
- Full-mouth or bitewing X-rays (when due)
- Routine dental cleaning – Prophylaxis (2 per year)
- Topical fluoride varnish (2 per year)
- Limited exam with 1 X-ray (1 per year)
- 10% courtesy on basic and major dental procedures
(For patients without gum disease)

- The annual fee must be paid in full at the time of enrollment.
- Payment methods accepted: Cash or Credit Card.
- The 10% courtesy discount does not apply to products or third-party financing.
